This form was too long for my little scanner, so I scanned the top and the bottom, and then merged them.  I'm not terribly thrilled with the results, so I've given the original top and bottom as well.  Conceivably I'll take it over to the scanner at Staples someday, but probably not.

In the Matter of the age of Sibyl G. Hosking, of the Township of Eramosa, in the County of Wellington, Province of Ontario, Dominion of Canada.

I, HENRY S. HOSKING, 

of the Township of Eramosa in the County of Wellington, Province of Ontario, Dominion of Canada,

Do Solemnly Declare that
  • 1. THAT I am the father of the said Sibyl G. Hosking.
  • 2. THAT the said Sibyl G. Hosking was born at the Town of Grand-Mere, Province of Quebec, Dominion of Canada, on the Twenty-seventh day of August 1897.
  • 3. THAT it is impossible to provide the regular certificate of birth as the Province of Quebec did not issue them at that time.

And I make this solemn declaration conscientiously believing it to be true and knowing that it is of the same force and effect as if made under oath and by virtue of "THE CANADA EVIDENCE ACT."

3. THAT it is impossible to provide the regular certificate of birth as the Province of Quebec did not issue them at that time.
I assume that Sibyl needed a birth certificate in order to get a Canadian passport so that she could go to Angola.  I imagine that she had been to the US before, since Hamilton is near the US border, but that she wouldn't have needed a passport for that.

Google AI tells me:
​The province of Quebec officially began civil registration of births on January 1, 1926, although church records (baptisms) served as the primary legal proof of birth before this date. Full civil, government-issued registration for all vital events did not become mandatory until January 1, 1994.

I wonder how much this declaration seemed like a routine occurrence to the Hoskings.  Not terribly routine, because apparently Sibyl reached the age of 26 without needing it, but not unheard-of, either.
